If I buy 37's and find a 4.10 or 4.88 gear swap I need this for the install right?

This kit can be used for any potential gear swap on a Ford Ranger with an M220 rear end. Just keep in mind that there's not a front-end install kit or gears yet for the IFS Clamshell/M190 front end. If you have an RWD Ranger with an M220, this install kit can be used with either 4.10 or 4.88 gears.

Excuse my ignorance, but how exactly will this differ than stock?

This particular kit is just an install kit for the rear end for folks that go with shorter/different rear-end gearing or an aftermarket differential. This kit doesn't really "differ" from stock. It comes with new differential hardware, since it's next to impossible to not damage the factory bearings and races when removing the stock differential or ring and pinion gears, not to mention setting backlash without a shim set.
